The Opportunity:

As our Care Coordinator Manager, you'll play a pivotal role in leading and shaping our care coordination practices. You'll oversee a team of specialists, developing and implementing processes to ensure seamless care transitions for patients across all departments and services.

Key Responsibilities:

• Lead and empower a team of care coordinator specialists, providing guidance, support, and development opportunities.

• Design and implement robust care coordination protocols for both wellness and illness scenarios, maximizing patient outcomes.

• Oversee the efficient flow of patient care activities across various departments and specialties.

• Continuously monitor and evaluate care coordination performance, identifying areas for improvement and driving change.

• Foster seamless patient experiences by collaborating effectively with healthcare providers, patients, and families.

• Stay up-to-date on industry best practices, regulations, and standards related to care coordination.

• Cultivate a culture of continuous improvement, prioritizing exceptional patient care.

• Ensure compliance with all relevant healthcare regulations and standards.

• Represent NEOM in industry and cross-sector meetings, fostering collaboration with key stakeholders.

• Manage stakeholder feedback, identify challenges, and develop action plans to address unmet needs.

• Contribute to internal and external communication initiatives.

• Lead by example, mentoring and coaching team members to achieve peak performance.

We Are Looking For:

• A seasoned healthcare professional with a minimum of 15 years of experience, including a strong background in nursing and care coordination.

• In-depth knowledge of nursing practices, healthcare procedures, and care plan development for both wellness and illness.

• Proficiency in healthcare regulations and quality standards.

• Expertise in monitoring and evaluating care coordination activities.

• Strong project management and leadership skills to guide and develop a team.

• Exceptional communication skills to connect effectively with healthcare providers, patients, and families.

• The ability to solve problems, collaborate with diverse stakeholders, and demonstrate compassion and empathy.

• Excellent time management skills to manage a demanding workload.

• A Bachelor's degree in Nursing or a related field.

• Proven leadership experience in managing and developing healthcare teams.

• A solid understanding of care coordination principles and best practices.

• A commitment to continuous learning and professional development.

• Experience with healthcare operations is a plus.
